If you would like to fax an order form you can do that with the form below. Please make sure you fax the 2 pages of the order form. No cover page is necessary.
All prices on the order form are in Canadian Dollars. USA customers can also order with the order form...however, please note that you credit card will be charged in Canadian funds.

International Customers can also use this form. Please fill out the form and leave the shipping and credit card info blank. Fax us the form with an email address that you can be reached at. Once we calculate the shipping and handling costs, we will email you the total cost. Complete the form with the charges and your credit card information and we will ship it out to you. Please note that most international orders will take 3-4 weeks as we ship via parcel post.
